The State of New York requires certain health-related screenings to be completed in our schools each year.  The requirements are as follows:

For New Entrants to the District:

     *Vision: distance acuity, near vision, hyperopia & color perception  

     *Hearing  

     *Physical exam-to be completed within 30 days of entry  

Vision Screening: (distance acuity only)

     -Kindergarten and grades 1, 2, 3, 5, 7, and 10  

Hearing

     -Kindergarten and grades 1, 3, 5, 7, and 10   

Scoliosis (Spinal Screening)

     -Grades 5 through 9  

Physical Exam (Health Appraisal)

     -PreKindergarten or Kindergarten and grades 1, 3, 5, 7, 9, and 11.  

If there are any unusual findings or cause for concern, the school nurse will contact the parent/guardian.  In the event additional evaluation is needed, a referral form will be sent home to the parent/guardian to take to their private healthcare provider.
